2. The promo period is from October 01, 2022 to September 30, 2023.
3. Qualified cardholders are entitled to the following:

• 10% off on Prepaid for the first transaction. Applicable for new customers. One time use only.
• 5% off on Prepaid Plans for succeeding transactions. Applicable for existing customers. Multiple use.

4. Purchase must be made using an eligible Bank of Commerce Credit and Debit Card within the promo period.

HOW TO AVAIL OF THE DISCOUNT

1. Visit www.mariahealth.ph
2. Click “I’m an Individual” tab on the upper portion.
3. Choose Prepaid Plans
4. Select your preferred Prepaid Plan and click “Buy Now” then “Add to Cart”.
5. Input the Promo Code. Discount should reflect immediately.
6. Click “Proceed to checkout”
7. Click the Shopping Cart icon on the right, topmost portion of the screen to view all the items on your Shopping Cart.
8. Accomplish the Payor’s contact details.
9. Click “Proceed to Payment”
10. Select “Credit or Debit Card” as form of payment then click “Checkout”.
11. Input your valid Bank of Commerce Credit or Debit Card details.
12. The e-voucher will be sent to your registered email address within 24 hours.
